Good Faith Estimate & the No Surprises Act
Dr. Blaising is in compliance with the No Surprises Act and will provide you with a Good Faith Estimate.
Services are both client-centered and client-driven. While Dr. Blaising may offer suggestions and encouragement about your treatment plan, she empowers her clients to make their own decisions regarding much of the scope of treatment. This includes but is not limited to the frequency of their sessions, the duration of treatment, and the breadth of issues addressed.
As we begin working on these issues, the above service is likely to take place on a weekly basis. However, you will be determining whether this frequency is increased or decreased and for how long you continue to receive treatment. The total cost of treatment is dependent on the number of sessions you decide to schedule multiplied by the rate for the service. The total will vary substantially from client to client even when the same diagnosis code is present.
If you have chosen to continue to receive services as we approach the end of the calendar year, we will revisit this Good Faith Estimate and make any appropriate adjustments based on your choices regarding your treatment and any changes that are made to the rates for services for the coming year.
